  • Login to UKG from the web and navigate to Menu > Myself, then scroll down to "Giving"

Frequently Asked Questions

Can I give by Credit Card? 
Yes, it is very easy to give by credit card and the link is provided when you select “Donate Today”.  There is a processing fee of 3% that is charged to the fund by the credit card company. You still receive credit for your full donation amount.  (I.e. You give $100 and $97 goes to fund and $3 to credit card processing fee)

Can I give by personal check and who do I make my check out to? 
All gifts for AxiaCares will be made directly to Provision Bridge.  Indicate "AxiaCares- 283065" in the memo line.

How much should I donate?
Donations of any amount are very much appreciated! If you are looking for a few ideas, consider the costs many are facing and you'll see that any gift makes a difference!

• Helps a colleague pay for temporary living when hit by a tornado
• Allows a colleague and child to avoid eviction
• Enables a colleague to feed his/her children a healthy meal after facing unexpected expenses
• Prevents water from being turned off in an apartment
• Even a “$5 a paycheck” recurrent contribution can go a long way to help and can be made easily via UKG

Can a gift be made from a Corporate or Individual Donor Advisor Fund (DAF)?
Yes. In order to donate from a donor-advised fund (DAF), the foundation usually either wires the funds, mails a check, or issues an ACH. If EIN# and contact information is needed please use the following:

Are donations tax deductible?
Yes. Donations are tax deductible as allowable by law.  You will receive confirmation of your gift from Provision Bridge, a non-profit organization.  Please save this for tax purposes.  In addition, if your gift is $250 or greater, you will receive an end of year letter for tax purposes.

Can my donation be directed to a specific area or care center?  
No. In order to qualify as a tax deduction all gifts will be directed to the AxiaCares fund which is intended to assist qualifying Axia employees across our organization.  We are all one Axia family and will try to assist those in greatest need.

How quickly will funds be distributed to those in need?  
It is the goal to make decisions each month on applications.  Please allow 30-45 days to be notified on whether your request can be granted.

How will it be decided who receives assistance from the AxiaCares Community Assistance Program?
Applications will be reviewed by the AxiaCares Review Committee to evaluate needs and determine funding amounts.

Will I have to provide financial documentation?
Yes, the Helping Hands charitable foundation may reach out to request additional financial documents based upon the amount of the grant request.

What is the range of funding one can receive?
Each situation is unique.  The AxiaCares Review Committee will do its best to maximize the amount of support granted to each employee based on the monies raised in support of the program.

If one is selected to receive assistance, will their grant amount be taxed now or in the future?
No, there are no taxes on distributions to an individual, or on payments made on their behalf.
